An implant depends on the bone and gum around it. If the bone has to be built up first, or gum disease treated, that happens here rather than becoming a referral and a wait. The crown on top is done by the general dentists you already see.

The Basics

What a Dental Implant Is

Implants are generally made of titanium and go into the jaw in place of the tooth’s root. There are three parts: the implant in the bone, an abutment through the gum, and the crown on top.

The bone grows around the implant and holds it there, a process called osseointegration. The American Academy of Periodontology notes that an implant replaces the root as well as the tooth, while a bridge or a partial denture leans on the teeth next to the gap.

Months, Not Weeks

Not a one-appointment treatment. The implant and the bone bond over about two to six months before the replacement tooth goes on, and a bone graft or sinus lift beforehand adds four to 12 months. Ask Dr. Mosquera for the schedule for your own case.

What Happens, Step by Step

Six steps, spread over months rather than weeks:

Step 1

Consultation and imaging

An exam and a scan: how much bone there is, where the nerves and sinuses sit.

Step 2

Any groundwork

Gum disease treated first; a bone graft or sinus lift where the bone is too thin.

Step 3

Placing the implant

A surgical appointment. Nitrous oxide or oral sedation is available if you want it.

Step 4

Healing

The bone heals around the implant, and a dentist may provide something temporary meanwhile.

Step 5

The abutment and the crown

Once the implant has integrated, your crown is fitted on top.

Step 6

Check-ups

The implant is monitored as part of your regular periodontal exam.

Suitability

Are You a Candidate?

Health matters more than age, according to the ADA’s patient library. What gets assessed:

  • Bone and gums. Enough bone to hold the implant, and gums free of periodontal disease.
  • General health. Chronic illness such as diabetes or leukemia can interfere with healing.
  • Diabetes. Not an automatic no: poorly controlled, above 8% HbA1c, does worse; well controlled, at or below 7%, often does as well as no diabetes.
  • Smoking. One meta-analysis of more than 150,000 implants found a 140.2% higher failure risk in smokers.
  • Bone medicines. Tell us if you take them. The ADA says they do not appear to rule an implant out.
  • Age. Health matters more than age, though in younger patients we wait for the jaw to finish growing.

Some of these change the plan or the order of treatment. Others can mean an implant is not the right answer, which is better said before treatment than after.

Groundwork

When a Bone Graft Comes First

When a tooth has been gone a while, the ridge is often left with too little bone to hold an implant. A bone graft fills that void with bone or a bone substitute, and in the upper back jaw a sinus lift does the same where the sinus sits close above the roots. Dr. Mosquera does both here.

The bone is then usually left to develop for four to 12 months before the implant goes in, so it is worth knowing at the planning stage rather than partway through.

Aftercare

Looking After Implants

An implant restoration is decay-resistant, but the American Academy of Periodontology is clear that an implant can still become diseased like a natural tooth. Across 102 studies, 46% of implant patients had inflammation of the gum around an implant and 21% had the form that reaches the bone. Caught early it is often reversible; once the bone is involved it usually needs surgery.

So the care is ordinary but it matters: brush and clean between your teeth as usual, keep your check-ups and cleanings, and tell us straight away if an implant feels loose or painful, or the gum around it bleeds. Implants can fail, and the FDA’s named risks include the abutment screw loosening and difficulty cleaning around the implant. One practical note: mention your implants before an MRI or X-ray, because they can interfere with the images.

Does insurance cover implants?

Plans differ widely: some cover part of the treatment, some the crown but not the surgery, some nothing at all. We are in network with Delta Dental/Washington Dental Service, Regence, Premera, Aetna and Cigna. Call the office and we will check your own plan with you.

Placing an implant is a surgical procedure, and Dr. Mosquera will go through what it involves before the day. Afterwards your dentist may suggest soft and cold foods while you heal. Nitrous oxide and oral sedation are available here if the appointment itself is the worry.

Months rather than weeks. The implant and bone bond over about two to six months before the replacement tooth is attached, and a graft or sinus lift first usually adds four to 12 months. You get the schedule for your own case at the planning visit.

A review of 18 studies put 10-year survival, counted per implant, at 96.4%, and its own more realistic analysis at 93.2%. Survival is not the same as success, which means the gum and bone around the implant stay healthy, so cleanings and check-ups still matter.
